Proposal Cover Sheet
All Student Research Fund Proposals must include an application form (also called a cover sheet). A separate cover sheet is used for Research and Presentation awards. Both cover sheets are available as a writable PDF.

Note: The PDFs are writable, but cannot be saved after completion. Complete the form and then print, sign, and place it on top of the full proposal.
Institutional Review Board
If your projects makes use of human beings as research subjects, it must be approved by the Institutional Review Board. A proposal which makes use of human beings but does not include this approval will not be considered. Before seeking approval please read the IRB guidelines.
Animal Care and Use
If your research project will be making use of animals you are required to submit the Animal Use and Care for with your proposal.
