Request a Transcript
To order an official Otterbein transcript, mail a written letter of request that includes the information below OR click here for a printable copy of the transcript request form to be completed and mailed.
- Full name (maiden or other name changes)
- Social Security number
- Year last attended or year graduated
- Student's legal signature (required by Federal law)
- Current permanent address
- Special instructions such as, place in a sealed envelope
- Payment by check or money order payable to Otterbein University; (we do not accept credit cards).
Rush Transcripts (cash and carry only)
A rush service is available at $10 per transcript. You must appear in person at our counter with a photo ID. This is a cash and carry only service. The transcript will be placed in a sealed envelope and given to you. A transcript cannt be issued if you have a financial hold on your account.
Cost
The cost of transcripts is $5. See above for rush transcripts.

IMPORTANT NOTE:
- It takes up to 5 business days to issue transcripts after the request is received; this can vary depending upon the time of year
- Transcripts cannot be ordered or received by fax or e-mail
- Transcripts cannot be released if you have an outstanding bill or an Otterbein loan in default
- Carefully complete all sections of the request form. Once your transcript has been issued, per your instructions, we are unable to reissue an official transcript without another request and additional payment.
- Unofficial transcripts are not available.
